I know you're anxious to hand over that credit card and get your itinerary finalized, but hold up! There's some very important information we need to get from you, and a few very important forms you need to sign, before we can send you the credit card authorization form for your payment.
No worries! There are only 3 forms below - and they are pretty quick and easy to do right from your phone or your computer!
(This is the FIRST form to fill out)
There are two options below - so you will need to choose the one that is applicable to the type of trip you are taking.
IMPORTANT - This form needs to be filled out for EVERY passenger going on this trip - yes, that includes children of all ages!
Parents - you can fill this form out on behalf of your minor children aged 17 and below, and you can use your email address and phone number on their form. Once you fill out the form for yourself just come back to this screen and click on the form again to fill it out for your child/children.
Each adult 18 and over will need to fill out their own form, with their own unique email address. (It will mess up my system if multiple adults all try to use the same email address on their forms!)
WHY do we ask you to securely upload a copy of your travel documents? So we can make sure that your information gets entered on your reservation correctly! If your name is misspelled, or if you are booked under a nickname versus your legal name you will be denied boarding at the airport or cruise terminal! Additionally, if your birthdate is listed incorrectly you may be denied boarding! (And you can't imagine how many times someone has entered their own name or date of birth - or the name or date of birth of a loved one - incorrectly on these forms!) Having a picture of your travel document allows us to verify names and dates of birth with the actual travel documents you will be using on the day of travel.
(Traveling with Driver's License and/or Birth Certificate)
If you are traveling domestically here in the U.S. - or if you are traveling on a closed-loop cruise that departs and returns to the same U.S. port (which doesn't REQUIRE a valid passport) - you can fill out this form.
If you are traveling with a passport, complete the PASSPORT ONLY section.
If you do not currently have a passport, complete the DRIVER'S LICENSE OR BIRTH CERTIFICATE ONLY section. (If you later obtain or renew your passport I can send you a form to just upload the passport information once you receive it in the mail.)

(Traveling Internationally which does require a valid passport)
As you may already know, when you travel from the U.S. to any international destination (including Canada and Mexico) you will be required to have a valid U.S. Passport.
NOTE: Many countries do require your passport to be valid for 6 months AFTER the date of your expected return.
Additionally, you will want to make sure that you have enough blank pages in your passport for your trip. Please visit https://travel.state.gov/content/travel.html for additional details on international travel requirements by country.

Please note that ALL adults age 18 and over who are looking to book travel with Creative Escapes, LLC must complete this form.
For minors age 17 and under at the time of booking, only one parent needs to fill out the form, and include their children's names and dates of birth at the bottom of their form.

Wise travelers recognize the important need to protect their trip investment, health, and personal belongings. Emergencies can happen when you least expect it, therefore, we highly recommend that you protect your vacation with travel insurance. Cruise lines, airlines, hotels, and tour operators all have penalties for cancellation, ranging from the full deposit to no refund at all. Also, keep in mind that most health insurance policies, including Medicare, may not offer medical coverage when you are out of the country, or once you have left US waters.
Travel Protection Insurance may offer protection in the following areas:
- Trip Cancellation
- Trip Delay
- Trip Interruption
- Missed Connection
- Baggage Loss, Theft or Damage
- Baggage Delay
- Emergency Medical and Dental Expenses
- Emergency Medical Transportation
- 24/7 Hotline Assistance
Many insurance suppliers do not provide the following coverage:
- Supplier Bankruptcy.
- Cancel for any reason (CFAR) coverage. (Meaning, your reason for cancellation must be a "covered" reason)
- Trip interruption coverage once you have departed.
- Medical coverage.
- 24-hour hotline assistance for travel and medical emergencies.
Even pre-existing coverage may not apply to world events such as strikes, natural disasters, terrorism, etc. It is your responsibility to obtain insurance BEFORE making the final payment for your vacation. While I am not a registered insurance agent and cannot legally provide specific advice or information regarding insurance, I can certainly provide you with options for you to review and choose from. Specific questions regarding policy coverage should be directed to the insurance provider.
On this form you will need to let me know how you'd like to proceed with regard to Travel Protection. Only one adult per reservation needs to fill out this form. If you are booking travel on behalf of others in your party, and you are making the decision on whether or not everyone on your reservation will or will not purchase travel protection, you will need to enter the names and birthdates of those individuals on the form, so we know on who's behalf you are making this decision.

Your Personal Travel Creator will be working to secure your reservation and place your itinerary on hold (if possible) with the vendor(s) they are working with.
When it is time to pay the deposit for your trip (or pay for it in full, if necessary) they will send you the credit card authorization form for your payment.
